Effect changes and fixes

• Gain or Lose a Heart: Added particles and a title to each outcome
• Anvil Apocalypse, Arrow Storm, & TNT Rain: Modified the way they spawn for slightly better functionality in the Nether (again) and spawn above water
• Arrow Storm: Each Arrow has 33% to be a Tipped Arrow, increased the amount of Arrows spawned, Arrows despawn from the ground after 1 second, and effect time increased to 60 seconds
• All Spawn Mob effects: Now the height at which mobs spawn is independent of where the player is looking
• Fixed 'Spooky Vision' sometimes not removing the Cursed Pumpkin when the effect runs out
• Fixed more effects still being displayed while 'Unknown Effects' is active
