Chunklocked Improvements

Renamed Chunklock UI text to Chunklocked. Added Chunklocked difficulty customization. Improved Nether and End support for Chunklocked worlds. Nether and End plots now use dimension-appropriate unlock requirements. Nether plots ask for Nether blocks, ores, and items. End plots ask for mob drops, End items, and later rare items like diamonds/emeralds. Fixed Nether/End portal spawning so players appear beside portals instead of inside them. Existing safe portals are reused instead of overwritten. Linked portal destination chunks now unlock correctly. Overworld portal returns now place players inside unlocked plots. Improved Chunklocked token HUD with a cleaner token guide. Simplified token meter rules: any natural block has a 50% chance to give +1 point. Removed damage from Chunklocked token progress.

Fixes

Removed the title screen subscribe badge. Kept the title screen mixin as a harmless placeholder. Fixed stale HUD info showing old damage and special ore point rules. Added/updated tests for token progress and dimension unlock requirements.
