Offhand transparency adjustments are now working whenever you're above or at Minecraft 1.21. Further injection into rendering for offhand items on 1.20.x is not feasible as I do not intend to write another render pipeline. In addition, Elytra, Armor and Skull opacity/transparency injections were verified on all versions and should now be working correctly (where previously some game/loader versions were only able to either fully hide or show).
Known Issues
Due to the nature of the injection into MC's rendering, especially shields and elytra cause a visual artifact when the item is partly transparent, for example a water texture behind the semi-transparent item will not be drawn anymore, and instead one is seeing the ocean floor. I can't think of a way to solve this currently.
