With this pack installed, End Altars will generate very rarely in the outer islands of the end. You can find them in survival just by exploring, but if you've got cheats enabled in your world, you can also type "/locate structure alt:end_altar"
The Altar accepts popped chorus fruit as a sacrifice. Dropping 32 popped chorus fruit (half a stack) on top of the froglight in the center will grant you a random item. You can also drop an entire stack (64 popped chorus fruits) to double the sacrifice, and receive twice the rewards.
The reward you receive from the altar is random - though some are rarer than others. Most of the time you'll receive something almost useless, like a chorus flower. Sometimes - though - you'll find a voided tool! Voided tools are made of a special type of diamond, making them unbreakable.
You can also find items like arrows of levitation, and spire armour trims.
Be careful not to damage the altar! Breaking blocks around it is fine, but if you destroying the froglight block will permanently break the structure - rendering it useless.
And as a last note, make sure you sacrifice exactly 32 or 64 popped chorus fruits, no more and no less! The altar will not accept sacrifices of 31, 33 or 63.
