- New sanity stat: exposure to the night, darkness, rain, and eating bad food will drain sanity; low sanity causes various kinds of hallucinatory and hostile shadow creature mobs to spawn along with a fog effect, brightened screen and screen-shake; sanity can be restored with certain foods, straw rolls, etc, sanity is visualized by a meter of brain icons
- Generation of saplings, boulders and grass tufts majorly rebalanced; saplings are now a bit more common, grass tufts now spawn excessively in savannas, and non-Rockyland boulders now generate more commonly; this is done to mitigate bad-start luck a bit more thoroughly
- New dimension: the caves dimension from Don't Starve (the caves dimension has three unique biomes: mushtree forests, sunken forests and cave marshes)
- New biome: graveyards (sparse brown forests with graves and heavy fog)
- New mobs: butterflies, fireflies, mandrakes, catcoons, ghosts, tentacles, batilisks and shadow creatures (mr. skitts, night hands, shadow watchers, crawling horrors and terrorbeaks)
- Improved tumbleweed spawning, the spawner block now checks if the area is loaded and if the player is actually in a desert
- Well Fed effect added, some foods give this; Well Fed prevents idle hunger drain and sprinting exhaustion (very filling foods like meaty stew do not have any threshold requirement, while moderately filling items such as bacon and eggs, honey ham, cooked beef, rabbit stew, etc require ten hunger or more to give Well Fed and mildly filling items such as bread, cooked chicken, baked potatoes, jerky, etc only give Well Fed at 15 hunger or above)
- Drying racks: put raw meat or kelp into it; said items will dry into jerky or dried seaweed over 1-2 days
- Crock pots: they take four input ingredients and take several seconds to a minute or two to produce crock pot dishes, which provide more significant health/hunger/sanity benefits than raw or cooked food alone
- Lanterns: the wooden lanterns from Don't Starve, lanterns consume fuel when placed (refuel them with fireflies or light bulbs), and they can be switched on/off by sneak-clicking
- Pumpkin lantern: craftable using pumpkins and fireflies, pumpkin lanterns emit light at night
- New ice item to match Don't Starve: ice, packed ice and blue ice now respectively drop 1, 2 and 3 ice if the tool doesn't have silk touch and if the replace-drops is enabled
- Straw rolls: portable one-use bed items that also restore sanity but drain ten hunger when used
- Backpacks: portable storage made from cut grass and twigs, backpacks can store nine items
- Miner hats: a new head-slot item that can ward off Charlie, also emits light if dynamic light is enabled
- Held torches (as well as held lanterns and equipped miner hats) protect the player from Charlie now if dynamic light is disabled
- Tail o Three Cats: craftable melee weapon that can pacify angered neutral mobs like wolves and endermen
- Darts: strong ranged projectile items made using Flint, cut reeds and feathers
- Log suits have been majorly nerfed to match how much durability they have in the DS-base game, but they now should absorb 80% of incoming damage
- Bug nets: catch fireflies, butterflies or bees with this
- Anti venom: a cure for the poison effect, also provides an entire in-game day of poison immunity
- Coral blocks now drop a Don't Starve-style coral item if the tool doesn't have silk touch, and if the replace-drops is enabled, this is to facilitate making anti venoms easier
- Garland: crafted from flower petals, garlands provide minor but passive sanity regen
- Kindling item made from one Flint and two sticks, string, paper, dried kelp, pine cones, twigs, cut grass, cut reeds, petals or foilage added; used to restart unlit fire pits
